Nursing Homes
Our goal is to provide a seamless and convenient experience for residents in nursing homes, retirement homes, and hospitals.
The facility staff, power of attorney (POA), or resident (if self-consenting) will send a referral to our office via phone, fax, or email. The referral should include the resident's name, facility name, POA's name and contact information, and a brief description of the denture concern.
Our team will reach out to the POA or resident (if self-consenting) to discuss the denture concern, relevant dental history, and fees for consultation or treatment. We will also obtain informed consent and schedule the first appointment, typically within 1-5 business days. The care facility will be notified of the appointment date and time.
A licensed denturist will meet the resident at the care facility, usually in their room. The denturist will perform a thorough examination, discuss treatment options, and obtain informed consent if not already provided. If consent is given, necessary procedures will be carried out.
If multiple appointments are needed, we will notify the POA and facility staff prior to each visit and provide updates on treatment progress.
We at SAL Direct Denture Care will handle all billing and paperwork directly with the POA or resident (if self-consenting), avoiding additional administrative workload for the care facility.
Upon completion of the treatment program, the resident, POA, and relevant staff will receive instructions for denture care, information on common issues to monitor, and guidance on contacting our office should any concerns arise.
Any necessary adjustment appointments within the first 3 months will be provided at no additional cost in most cases.

For patients receiving care in their homes, our process is designed to be convenient and efficient:
The patient, power of attorney (POA), or substitute decision maker (SDM) can reach out to our office via phone, fax, or email to schedule an appointment for a consultation or proposed treatment. Appointments are typically scheduled within 1-5 business days.
A licensed denturist will visit the patient at their residence, providing a thorough examination and discussing treatment options and associated costs with the patient, POA, or SDM. In most cases, treatment can begin during this initial visit.
Any necessary follow-up appointments for the treatment plan will be scheduled directly with the patient, POA, or SDM.
After completing the treatment program, the patient, POA, or SDM will receive detailed post-care instructions, including how to contact our office should any follow-up adjustments be needed. Most adjustment appointments within the first 3 months are provided at no additional charge.
